Aortic root surgery in Marfan syndrome: Bentall procedure with the composite mechanical valved conduit versus David reimplantation with the Valsalva graft.

We read with interest the article by Nardi etal. [1], which compares Bentall versus reimplantation technique in Marfan patients. This article deserves particular attention considering the limited published surgical experiences with valve-sparing technique using the Valsalva graft in the Marfan subgroup. This statement is not only related to absolute numbers of patients globally reported by the authors, but mostly to the number of cardiac surgery centers trained for the use of such a prosthesis. As a matter of the fact, we do think that the authors correctly described the technique as 'aortic valve reimplantation with valsalva graft'. In our opinion, the use of a prefashioned Valsalva graft does not allow us to define the technique as 'David F, as the original technique described by Feindel employed a straight graft.
