Inhaled milrinone and epoprostenol in a patient with severe pulmonary hypertension, right ventricular failure, and reduced baseline brain saturation value from a left atrial myxoma

MYXOMAS ACCOUNT for approximately half of benign heart tumors.1 The signs and symptoms associated with myxomas are related closely to their size and location.1'2 Although large left-sided atrial myxomas may remain asymptomatic, they sometimes can be responsible for a spectrum of severe complications evolving from left atrial outflow obstruction, left ventricular failure, pulmonary hypertension (PHTN), and, rarely, right ventricular (RV) failure.1"3 The authors report the preemptive use of inhaled milrinone and epoprostenol for the intraoperative management of a patient with left atrial myxoma associated with severe PHTN and RV failure.
