Which protein C after cardiac surgery?

We read with great interest the case report by Hudcova et al1 regarding the use of activated protein C (APC) in a patient diagnosed with septic shock, coagulopathy, and endocarditis immediately after cardiac surgery. The favorable outcome despite the absence of massive bleeding and hemorrhagic complications seems to support the use of APC even in the presence of relative contraindications, especially in patients with tube drainages and open cavities/wounds. Two years ago, Pappa-lardo et al2 reported the uneventful use of APC in 2 septic cardiac surgical" patients (started the 6th and 5th day after surgery, respectively) simultaneously receiving warfarin and heparin with rapid recovery from septic shock.
