Wide-Field Fundus Imaging Using the Optos Optomap and a Disposable Eyelid Speculum

The EzSpec (a disposable eyelid speculum recently designed for intravitreal injections) is a useful device for removing eyelash artifacts and for potentially increasing the sensitivity of the Optos Optomap system, thereby improving the detection of retinal lesions. The Optos Optomap consists of a scanning laser ophthalmoscope that can obtain images without mydriasis. This imaging system was designed to provide a 200° field of the retina. Consequently, the wide-field fundus photography enabled by this system can provide images of sufficient retinal area to document a variety of retinal lesions, such as retinal tears, retinal detachment, and proliferative diabetic retinopathy. However, Mackenzie et al reported that the sensitivity of the Optos Optomap was lower for lesions anterior to the equator than for lesions posterior to the equator (45% vs 74%) because of a reduction in image quality and because areas in the extreme periphery could not be imaged. Furthermore, they pointed out that eyelash artifacts prevented the clear imaging of the inferior periphery.
