Single-stage turn-over muscular hinge flap with burow's full-thickness skin graft to repair oral commissure defect

A 73-year-old Caucasian man presented with a recurrent squamous cell carcinoma of the right oral commissure that had been present for at least 4 years. The tumor was excised using Mohs micrographic surgery, requiring two stages to achieve a tumor-free plane. The postoperative defect was deep, vertically oriented, and measured 4.0 by 1.5 cm; it resulted in skin, muscle, and mucosal loss of the upper and lower lip and the right oral commissure. The wound crossed the vermillion border of the upper and lower lips and included wet and dry mucosa (Figure 1A, B). How would you reconstruct this defect?
