Staged trilobe pedicle flap to repair a large cutaneous lip and nasal vestibule defect

A 68-year-old Mexican man presented with a left upper lip basal cell carcinoma that had been present for 2 years. The carcinoma was excised using Mohs micrographic surgery; five stages were required to achieve a tumor-free plane, leaving a postoperative defect that measured 2.8 x 4.4 cm (Figure 1). A small portion of the defect was through-and-through in the lip above the vermillion, and the wound included the nasal sill, a portion of the nasal vestibule floor, and the underside of the proximal ala. How would you repair this defect?
