Human factors analysis for process safety

OSHA's Process Safety Management (PSM) standard, 29 CFR 1910.119 and EPA's Risk Management Program (RMP) rule, 40 CER Part 68 require that a process hazarde analysis (PHA) be performed for processes covered by the regulations. The PHA must address both human failures that can cause accidents, and human factors (HF) that can influence them. Examples of HF issues that OSHA is concerned about, and that can contribute to hazard scenarios, include controls and instrumentation accessibility, clarity and usability, task overload and work schedules.
