The death of a worker struck by a piece of timber used to cushion a sewage pipe as it was pushed into position by an excavator could have been prevented if his employer had involved staff in its risk assessments. Fredric March and two colleagues were replacing several hundred metres of sewage pipes across a field after a pipe burst near Galton Way, Swindon, on 10 June 2011.
