"BP GETS it. And I get it too." These were the words of Lord Browne, then chief executive of BP, when an independent US review criticised the oil giant's safety culture after an explosion at its Texas City refinery killed 15 people almost eight years ago. The review panel condemned the firm's London-based board for its inadequate leadership and a corporate "blind spot" that led it to focus on personal safety at the expense of process safety. Browne promised that he and his successor, Tony Hayward, would take the lead in "championing process safety as a foundation of BP's operations" and Hayward later promised to focus "like a laser" on safety.
