WASTE management firm SITA has been fined 180,000 pounds after an employee was crushed to death inside a cardboard baling machine he hadn't been trained to use. Paul McGuire was fatally injured in August 2005 while using the machine at SITA-operated Kingshill Recycling Centre in Cricklade, Wiltshire. He was feeding paper and cardboard into the baler when it jammed. The blockage was at the top of the machine's conveyor, so McGuire, who hadn't been formally trained or authorised to use the machine and was operating it alone, climbed up the conveyor to try to clear it. He fell into the hopper. The machine hadn't been isolated or locked off and McGuire was killed by the baler's mechanism. An inquest in April 2007 recorded a verdict of death by misadventure.
