Dialysis is undoubtedly one of the major achievements in modern medicine. However long-standing treatment of end-stage renal disease (ESRD) with dialysis is associated with high mortality, the five-year survival of ca. 50 % being only marginally better than in lung cancer. Although increasing small-molecular weight protein clearance (Kt/V) over a certain threshold does not appear to improve results as shown in the HEMO study [1], high-volume hemodiafiltration may positively influence survival according to recent reports [2]. Since only minority of the ESRD patients ever receive a kidney transplant the dialysis population is expanding continuously: today there are ca. 260.000 dialysis patients in Europe and close to 400.000 in the US. Thus current the ESRD therapy has a very serious impact of health economy expenditure.
