A 14-year-old girl with hip pain and fever after a skateboard fall

Her past medical history was significant for an uncomplicated laparoscopic appendectomy in early childhood and ana-phylaxis with penicillin. Physical examination revealed a temperature of 38.8°C, pulse of 112 beats/min, blood pressure of 107/50 mmHg and respiratory rate of 22 breaths/min. She was a well-nourished girl who was lying supine and motionless on the bed because any slight movement caused her severe discomfort. Abdominal examination revealed tenderness throughout with voluntary guarding localized over the lower abdomen. Psoas sign was positive on the left. Pain in the left lower abdomen, presacral region, lateral trochanter and buttock was worsened by straight leg raise and caused radiation to her foot.
