A false-positive V/Q scan due to systemic arterialization of lung

A 57-year-old woman was recently discharged from the hospital with a diagnosis of pulmonary embolism (PE) that was established by ventilation-perfusion (V/Q) scan. She presented with similar symptoms 1 week later. Angiography demonstrated anomalous, systemic blood supply to the right apex. No evidence of PE was seen. This is a case of systemic arterialization of normal lung, which should be included in the differential diagnosis in patients with false-positive high-probability (V/Q) scans.
