Onychomadesis secondary to penicillin allergy in a child

A23-month-old boy presented with a 3-day history of headache, sore throat, and cough. He experienced no fever, vomiting, or diarrhea. On examination, his tonsils were enlarged and red. Most of the child's fingernails were in the process of sloughing off and being replaced by new growth (Figure). AH toenails and fingernails were affected and some of the nails had already dropped off.
