Riddick et al. provide useful validation of a technique to try and avoid dorsal cortical penetration and thus extensor tendon irritation and rupture following fixation for distal radius fractures. The concern is that even with dry bones and optimum positioning correct estimation of penetration was 83%. This adds to the increasing volume of literature on dealing with the complications of distal radius fixation using fixed angle volar locking plates. The increase in number and type of complications parallel the increasing use of such plates for dealing with distal radius fractures which represent a significant workload to health services, particularly in areas with an aging population.
