Dorsal metacarpal artery perforator-based propeller flap for complex defect of the dorsal aspect in the index finger

Dear Sir,A 43-year-old man was admitted to our department with a lesion to the index finger caused by a circular saw. He presented with a complex defect involving the dorsal skin and extensor tendons from second MP to DIP joint, as well as a partial bone loss of the dorsal aspect of the proximal and middle phalanx (Fig la). The patient underwent an emergency operation to repair the defect by using a composite teno-fasciocu-taneous flap based on a perforator branch of the second dorsal intermetacarpal artery. The perforator artery was 1 cm proximal to the second MP joint. The flap was 8?5 cm large and included the extensor indicis proprius tendon from extensor retinaculum to second MP joint.
