Intraosseous xanthoma of the hand without an underlying lipid disorder

A 37-year-old man presented with 5 month history of a non-tender swelling in the distal phalanx of the non-dominant little finger, without any previous history of trauma or infection, past medical history or relevant family history. Examination showed a clubbed appearance of the finger, without sensory deficit and intact flexor and extensor mechanisms. Radiographs identified a lytic, destructive lesion in the tip of the distal phalanx, sparing the interphalangeal joint (Figure 1).
