Graves Ophthalmopathy After Treatment of Subclinical Hyperthyroidism From Multinodular Goiter With Radioactive Iodine

The development of thyroid stimulating hormone receptor antibodies (TRAB) and associated Graves disease has been previously described in patients with multinodular goiter after treatment with radioactive iodine. We describe the case of a 72-year-old man with subclinical hyperthyroidism secondary to multinodular goiter who developed TRAB and Graves ophthalmopathy after treatment with radioactive iodine. The patient had become hypothyroid, when his eye disease first made its appearance. This appears to be the first reported case of isolated Graves ophthalmopathy in a patient who was hypothyroid at the time of presentation and who developed TRAB after treatment for multinodular goiter.
