Author's reply: Randomized clinical trial of pancreaticogastrostomy versus pancreaticojejunostomy on the rate and severity of pancreatic fistula after pancreaticoduodenectomy (Br J Surg 2013; 100: 1597-1605)

Sir We appreciate the comments offered by Dr Goh regarding our recent randomized clinical trial on pancreatic anastomosis. We are in agreement that institution, surgeon volume and experience are key factors in pancreatic surgery. The two hospitals participating in the study are university hospitals with a wide experience in pancreatic surgery and the patients are treated by a multi-disciplinary team. Both hospitals had carried out at least 15 pancreatic resections before starting the study with the two anastomotic techniques described. These two types of pancreatic anastomosis were selected specifically. The duct-to-mucosa pancreaticojejunostomy (PJ) and the invaginated pancreaticogas-trostomy (PG) were considered as the most frequently used anastomoses in the literature. Only one type of PJ anastomosis could be allowed in the study for the homogeneity of that group. The main authors of the paper (J.F. or L.S.) performed all the procedures personally or assisted the surgeon.
