We read the article by Menees et al. (1) with great interest and commend the authors for investigating an area in gastroenterology where procedures may be overused and physicians may not be adhering to recommended guidelines. In an era of quality, safe, and cost-effective medical care, inappropriate procedures are not acceptable. However, we take issue with a major premise of the authors' methods and resultant conclusions. The authors separate bowel preparations into poor verses any other prep categorization with a poor prep necessitating a repeat colon-oscopy within 1 year and any other prep necessitating a repeat colonoscopy in 10 years (1). This assertion is not consistent with the current multisociety guideline on colon cancer screening, as approved by the American Gastroenterological Association, American College of Gastroenterology, and American Society of Gastrointestinal Endoscopy (2).
