Genital melanocytic naevus and lichen sclerosus

Vulvar melanocytic proliferations on a background of lichen sclerosus may mimic features of malignant melanoma clinically and pathologically. We present a case of an enlarging genital naevus in a prepubescent girl with lichen sclerosus and discuss the association of the two conditions.A 7-year-old girl was referred to a gynaecological oncologist with a pigmented vulvar lesion, discovered by the girl's mother who inspected the vulva due to her daughter's complaint of pruritis. Symptoms had been present for about 6 months. No family history of melanoma or lichen sclerosus was elicited. Examination revealed a 6 x 3 mm darkly pigmented lesion on the left labium minus lying within an area of symmetrical pallor involving the clitoral hood, bilateral labia, and perineum (Fig. 1). Clinical diagnoses of a benign pigmented lesion and lichen sclerosus were made. A paediatric dermatological consultant reviewed the girl and agreed with these diagnoses. No biopsies were performed.
