Evans Syndrome (immune-mediated haemolytic anaemia with immune-mediated thrombocytopenia) in a three-year-old male neutered Springer Spaniel

A three-year-old male neutered Springer Spaniel presented with a profound regenerative anaemia. Spherocytosis and in-saline agglutination confirmed the presence of immune-mediated haemolytic anaemia and the dog received immunosuppressive steroids anda packed red cell transfusion. Over the course of the following 48 hours further red cell destruction was seen, as well as a marked fall in platelet numbers, suggesting the presence of immune-mediated thrombocytopenia - Evans syndrome. No underlying trigger could be found, suggesting primary immune-mediated disease, and the dog made a full recovery with a tapering six-month course of immunosuppressive steroids and azathioprine.
