Profound hypertriglyceridemia and weight gain in the first week following initiation of olanzapine: A case report with implications for lipid monitoring guidelines

Cases of severe hypertriglyceridemia related to olanzapine began appearing more than a decade ago. Meyer documented a case series in 2001, with a time to peak elevation that ranged from 2 to 23.5 months. A significant number of prospective trials followed and confirmed the risk of hypertriglyceridemia in patients on certain atypical antipsychotics, especially those in the dibenzodiazepine class. Though there are several prior studies that document an increase in triglycerides within the first month of atypical antipsychotic treatment, consensus guidelines often suggest follow-up lipid monitoring no sooner than 12 weeks after medication initiation.
