The use of neuroimaging is of uncertain value in the routine evaluation of new-onset psychiatric disorders. We present the case of an apparent postpartum depression with psychosis in a woman with a history of childhood acute lymphoblastic leukemia (ALL) treated with central nervous system (CNS) radiation. Neuroimaging during an inpatient psychiatric hospitalization revealed a large bi-frontal meningioma. Patients treated with CNS radiation for childhood cancer may be a subset of patients in whom neuroimaging is warranted in the setting of new-onset psychosis.
