Mr. S is a 59-year old male with paranoid schizophrenia since his late 30 s. Since then he was treated with a conventional antipsychotic (pipamperone 80 mg/day). At his presentation in our department he had abnormal movements of the jaw (biting, chewing and mouth opening), lips (puckering and smacking), and tongue, and frowning, blinking and grimacing of the upper face, which were reported to be present for at least 18 months. The Abnormal Involuntary Movements Scale (AIMS) score at that time was 21. It was decided to discontinue the conventional agent and to initiate ziprasidone (80 mg/day). The first DAT scan was performed before the initiation of ziprasidone and under pipamperone treatment Tomographic (SPECT) imaging of the brain was performed 3 hours following intravenous administration of 5 mCi of I-123 FP-CIT. The DAT scan revealed relatively decreased tracer uptake in the left basal ganglia compared to the right basal ganglia (the ratio of mean pixel value of ROI (region of interest) over the basal ganglia/mean pixel value of ROI over background (occipital lobe) was 1.51 and 1.47 for the right and left basal ganglia respectively).
