The second edition of Heinrich's book "Industrial Accident Prevention: A Scientific Approach" was published in 1941. Not far removed from that time, H.L. Mencken suggested something like, "For every complex problem, there is a solution that is simple, neat and wrong." Injury causation is a complex loss-control problem. Heinrich offered his falling-domino explanation to this complex issue in 1941, and it was - and remains - simple, neat and wrong.
