Challenges for Risk Management Approaches in the Offshore Industry in the Modelling of Human & Organizational Factors' Influence on Risk

The Macondo deep-water blowout in 2010 in US GoM has demonstrated the need for a risk-based (or risk-informed) approach to risk management, in fact the Presidential Commission pointed that out in their investigation of the Macondo blowout, and pointed to North European practices. Yet no changes to legislation have been made so far. Risk-based approaches have been utilized in the offshore oil and gas sector in Norway for more than 30 years, in UK for over 20 years. Major accidents with fatalities have not occurred after Piper Alpha in the UK in 1988. But serious incidents and major accident precursor events continue to occur, also in these sectors. The objective of the paper is to present a brief overview of current applications of risk assessment and management approaches and techniques and discuss future challenges, especially in the Human and Organizational Factor (HOF) sphere.
