Requirements, recommendations, notices, guidelines, alerts - what can possibly go wrong?

In 2002, IMPA started a safety campaign on pilot transfer arrangements (PTA), a campaign that has been carried out every year since. The outcome of this survey shows a steady average of 20% non-compliance. It has been like that for the past 20 years and we do not see a lot of improvement in the numbers. Other surveys, and my own personal tally, show a staggering result of over 50% non-compliance. For the sake of this discussion, let us assume the 20% result of the IMPA campaign. But even if non-compliance were 5 or 10%, would that be acceptable in any other industry? Suppose every fifth plane were unsafe? Or every fifth nuclear or chemical facility?
