Responsibility for the 2007 Cosco Busan oil spill, which saw around 53,000 gallons of bunker oil spilled into San Francisco Bay in the United States when the vessel collided with the San Francisco Bay Bridge, was placed largely on US pilot John Cota, whose judgement was found to be impaired by the use of prescription drugs. A report into the incident, published by the California state Board of Pilot Commissioners, found that the incident took place because Cota was unable to correctly operate the onboard radar and electronic navigation charts as the Chinese-flagged container vessel careered at high speed through thick fog into the bridge.
