The grounding of general cargo vessel Claude A Desgagnes in the St Lawrence Seaway in 2013 was caused by a failure of the pilot and the ship's master to agree on how to slow it when approaching a lock, according to a report from the Transportation Safety Board of Canada. It noted that the ship was about 500m behind another vessel approaching the locks at Iroquois, Ontario. The pilot wanted an anchor lowered to slow the ship while the captain, who had just returned to the bridge, wanted to reverse the engines.
