Fatal Fire-Fighting Accident Attributed to Contractor/Reg Failures

The National Transportation Safety Board (NTSB) concluded that improper actions by the contractor and insufficient oversight by government agencies led to a 2008 helicopter crash that took the lives of seven firefighters and two crew members. The chopper was being operated by the U.S. Forest Service (USFS) to transport firefighters to battle fires when it crashed near Weaverville, California. USFS had contracted with Carson Helicopters Inc. of Grants Pass, Oregon.
