State concludes inquiry into zip line accident

Hawaii OSHA (HIOSH) has determined that an employee of a zip line operation was killed because soil in which ground anchors were installed could not withstand the load from the tower, cables, and rider. HIOSH says that the business, GoZip, failed to take reasonable precautions to ensure that the anchors could support the load. According to the state plan, the owner of GoZip helped write industry standards for installing zip line courses.
