HUMAN ERROR

The term 'human error' is often given as a cause of an accident, especially when other explanations for the event to have occurred have not been identified. And the way of addressing this 'human error problem' is generally thought to lie in either changing the people (or, at least, their behaviour) or their role in the system. For example, it might be argued that bringing in more automation could reduce the human role, or that human behaviour could be altered by regimentation such as introducing stricter monitoring, rules or procedures.
