Firms fined over 'entirely preventable' deaths

The Old Bailey heard that on 21 January 2011, a large steel structure the men were constructing as part of the foundation for a Pressure Test Facility (PTF) at Claxton Engineering Services Ltd in Great Yarmouth, collapsed while they were inside it. A large-scale emergency response took place to try and rescue them, however, Adam Taylor, Peter Johnson and brothers, Thomas and Daniel Hazelton, were all pronounced dead at the scene.
