Applying the Chronic Care Model to the Management of Obesity

Management of chronic conditions requires a ndifferent set of processes than care of acute dis-nease. Chronic conditions are usually lifelong in nduration, without permanent cure. People live with their nchronic conditions every day and spend most of their time nin their homes, workplaces, and communities rather than nin healthcare facilities. They become the principal deci-nsion makers, acting not as passive consumers but as active nparticipants. The management of chronic conditions, then, nrequires a partnership between a healthcare team and the nperson living with a chronic condition.
