L'ANAPHYLAXIE ALIMENTAIRE DE L'ENFANT ET SA PRISE EN CHARGE

L'objectif de cette revue est de roppeller aux praticiens le risque d'anaphylaxie alimentaire auquel est confronté l'enfant, ses complications parfois fatales, son traitement et les modalités de prise en charge qui accompagnent le diagnostic. L'enfant en âge scolaire est particulièrement exposé parce qu'il est plus indépendant que le nourrisson et moins attentif que l'adulte. Le syndrome d'allergie orale et l'anaphylaxie alimentaire induite par l'effort sont deux présentations particulières. Le diagnostic repose sur l'anamnèse, les tests cutanés et les IgE spécifiques. Le test de provocation en double aveugle est trop dangereux. La prévention de l'anaphylaxie alimentaire reste l'éviction de l'aliment, et l'adrénaline le médicament d'urgence des manifestations sévères.%The aim of this review is reminder of the risk of food-anaphylaxis in the young child, the ampliations, sometimes lethal, the treatment and supportive care after the diagnosis. Children at school are especially at risk because they are more independent than the infant and lea careful than the adult. Oral allergy syndrome and exercice-induced food anaphylaxis are two particular forms of food allergy. Diagnosis is based on history, skin testing and specific IgE. Double blind food challenge is too dangerous in case of anaphylaxis. Prevention is avoidance and adrenalin is the first treatment of severe clinical manifestations.
