INFORMATISATION D'UNE UNITE DE SOINS INTENSIFS : EXPÉRIENCE DE LA PHASE PILOTE AU CHUV. POINT DE VUE MÉDICAL

Before introducing a clinical information systems into the entire 44 beds ICU, a pilot phase was initiated in a small subunit (4 beds) to test the impact of the system on clinical practice. The implementation in unit used to paper work was mainly confronted with human problems related to teaching, adaptation, a change in culture and communication problems. The main restriction was the feeling of having lost the global picture of the patient despite the provision of a huge amount of informations. After a few weeks, it was observed that a more extensive standardisation of prescription schemes, and of nursing and therapeutic protocols was required. This was associated with the development of synthetic layouts associating informations from the various body systems. Such a development requires the dose collaboration with the system's computer specialists.%Pour introduire un système d'information clinique, le CHUV a choisi une petite unité de soins intensifs. Les objectifs prioritaires d'un tel projet incluent l'automatisation de la saisie des données, l'amélioration de la qualité des variables saisies, la gestion intégrée des informations et l'aide à la décision thérapeutique. Le paramétrage a permis d'adapter le système à nos pratiques de médecine intensive. La phase pilote permet d'évaluer l'impact de la démarche sur ces pratiques. Le reproche principal fait au système était l'impression d'avoir perdu de vue l'image globale du patient malgré la profusion d'informations. L'interface humaine s'avère être le principal problème : apprentissage, adaptation, changement de culture, et communication sont essentiels et difficiles à réaliser, et impliquent de la standardisation et du développement en particulier dans le domaine des pratiques de soins et des modes de prescription, ainsi qu'une collaboration étroite avec les informaticiens.
