Forensic Investigation of the Deepwater Horizon Blowout Preventer

On the evening of April 20, 2010, control of an exploratory well at the Macondo Prospect off the coast of Louisiana in the Gulf of Mexico was lost, allowing hydrocarbons to enter the drilling riser and reach the Deepwater Horizon (DWH) drilling rig. The subsequent fires and explosions onboard resulted in the rig sinking on April 22, 2010 and the loss of 11 lives. Hydrocarbons continued to flow from the well until September 19, 2010, when a relief well was completed.
