Human factors revisited

To err may be human but that sure isn't helpful when it comes to shipping safety and environmental protection. While great advances have been made in training, automation and making things more ergonomical, on average about 80% of marine casualties are caused, in some part, by human error. These errors can be the result of fatigue, inadequate communications, technical knowledge of equipment or ship systems, poor maintenance, poor weather conditions or poor systems design.
