It looks like we were a little previous in blaming poor housekeeping for the $400m fire aboard the nuclear sub USS Miami under refit at the Portsmouth Naval Shipyard in Maine back in May. Last months column blamed a faulty vacuum cleaner for sparking the blaze but it now transpires that there was a guiding hand behind it.
