There have been several air- line incidents and accidents involving professional pilots not realising, for whatever reason, that a parameter such as speed is dangerously incorrect. While Stephen Trimble's article ("Wake-Up Call For Cockpits", flight International 30 July) highlights an emphasis on better monitoring techniques, which would be no bad thing, there is also an observation that putting emphasis on [pilot] monitoring may, on its own, be insufficient.
