Indonesian investigators have concluded that controller errors, miscommunication and poor lighting conditions all contributed to a take-off collision at Jakarta's Halim Perdanakusuma airport last April in which the vertical and horizontal stabilisers of a towed ATR 42-600 were severed by the wing-tip of a departing Boeing 737-800.
