Use of topical 2.4% glycopyrronium tosylate in familial benign pemphigus (Hailey-Hailey disease)

Familial benign pemphigus (Hailey-Hailey disease) isan autosomal dominant genodermatosischaracterized by flaccid vesicles and bullae that giverise to erosions primarily in intertriginous areas [1].This disease is notoriously difficult to treat owing toits chronicity and relapsing-remitting course. Sweatis a known exacerbating factor and oralanticholinergics such as glycopyrrolate have beenused to target this component of the disease process[2]. Glycopyrronium tosylate 2.4% cloth, which isFDA-approved for the treatment of axillaryhyperhidrosis, is a topical therapeutic option tomanage patients with Hailey-Hailey disease given itsease of application and decreased risk foranticholinergic side effects [3]. In this report, wepresent a patient with Hailey-Hailey disease of thescrotum and bilateral inguinal creases who achievedsignificant improvement with GT 2.4% cloth. Thiscase demonstrates a novel, safe, topical therapeuticoption for Hailey-Hailey disease.
