Reliable and accurate detection of carbapenemase-producing enterobacteria (CPE) is based on both non-molecular (phenotypic) and molecular methods. The obvious first step is recognition of carbapenem resistance using clinical breakpoints (EUCAST/CLSI); however, as some susceptible enterobacteria can still produce a carbapenemase, screening criteria (as defined by EUCAST), whole pattern of β-lactam resistance and simultaneous resistance to other families should also be considered. Care should be taken when using automatic methods, which may produce false susceptibility results for CPE.
