Very-low-birth-weight (VLBW) premature infants show ahigh risk of developing surfactant deficiency, respiratorydifficulty, or apnea after birth. The neonatal resuscitationguideline recommends resuscitation in the delivery roomby positive pressure ventilation (PPV) to rescue high-risknewborns suffering from apnea, gasping, or a heart rate ofless than 100/min.1,2 Positive end-expiratory pressure(PEEP) using 5 cm H2O is recommended while applying PPVin neonatal resuscitation; however, there is still a lack ofstrong evidence of the beneficial significance of PEEP inVLBW infants through clinically randomized controlledtrials.
