Abstract：The community-based and home-based nursing system still in the primary stage of development. It can only provide basic nursing service, which may cause insufficient training in stroke patient after discharge and leading to the further damage of hemiplegic limbs and recurrent stroke. The set and operation of community-based and home-based nursing system in abroad is comparatively perfect. The whole rehabilitation team consists of rehabilitation nursing specialist, psychological therapist, dietician and other staffs from different medical areas. This paper is aim to review the current status of home-based rehabilitation nursing in stroke patient, and to summarize the nursing measures and related assessment tools, in order to provided suitable evidence for home-based rehabilitation nursing in stroke patient.
