Improved baPWV in Type 2 Diabetes after Short-Term Glycemic Control is Accompanied by Increased Levels of PTX3 and Decreased Levels of LPC

We previously observed improvements in brachial-ankle pulse wave velocity (baPWV) after in-hospital short-term glycemic control for type 2 diabetes mellitus (T2DM). To elucidate the mechanism of baPWV improvement in the context of inflammation and lipid metabolism, we assessed the contribution of Pentraxin 3 (PTX3) and lysophosphatidylcholine (LPC) to the changes in baPWV in such cases. The study was performed at the Japan Self Defense Forces Central Hospital. Thirty male T2DM patients admitted to the hospital for glycemic control were recruited after local ethical committee approval. In each patient, baPWV, blood pressure, blood sample of fasting glucose level, PTX3, γ-glutamyl transpeptidase (γ-GTP), LPC, and Angiopoietin-like protein 3 (ANGPTL3) were assessed at admission and at discharge. After glycemic control, baPWV decreased from 1452 ± 200 to 1311 ± 194 cm/s. There was a positive correlation between baPWV and γ-GTP after adjustment for the influence of blood pressure. In the context of inflammation, PTX3 increased after glycemic control (from 2.4 ± 1.6 to 3.9 ± 2.7 ng/ml). A decrease in plasma LPC was observed in each patient after glycemic control (from 360±61.4 to 300 ± 53.5 μg/dl); however, PTX3 and LPC did not correlate significantly with baPWV. The rate of PTX3 variability was positively correlated with that of ANGPTL3. A rapid increase in PTX3 and decrease in LPC after glycemic control was accompanied by weight loss and improved glucose levels. There might be a possible association between PTX3 and ANGPTL3. We also demonstrated a positive correlation of baPWV with γ-GTP in short-term glycemic control in each individual.
