An Overview of the Medical Home Clinic for Special Needs Children at a Tertiary Care Center in Regard to the Role of the Registered Dietitian

Medically complex children are children with special needs with the most medically complex conditions. The Medical Home Clinic for Special Needs Children provides care for these children through an outpatient clinic using the premises of the medical home concept of care as defined by the American Academy of Pediatrics. Care is provided by a multidisciplinary team, which works together with the caregivers and patients and assists in care coordination and management. A registered dietitian is an essential member of the interdisciplinary team to help manage nutritional needs to promote proper growth and development and to help ensure patients and families have access to nutritional supplies to meet the child’s needs.
