As I scan the list of sources for pilot/controller confusion in the accompanying article, it's deja vu all over again. I've made more than a few of those same mistakes in my 40 years as a pilot, and as a controller at O'Hare, I watched a lot of other folks do the same. For instance, I not only saw controllers issue go-around instructions to aircraft that were already on the ground, I saw pilots comply with those instructions. You'd be surprised how many inadvertent touch-and-goes are made by the big iron, usually because of a botched landing, but occasionally in blind obedience to a beleaguered controller trying to correct a separation situation far too late in the game.
