This is the twelfth of a series of articles that will review historical ARFF responses with significant lessons to learn. We will try to read between the lines of accident reports, magazine articles, conference presentations, pictures, and other information sources to attempt to determine what problems the emergency responders encountered, as well as their resulting size-up, action plan, and end results. We will try to learn from the past, in order to be able to perform better in the future. The accidents that will be reviewed in this series of articles can easily happen again.
